Highly individualised support that’s tailored to your needs, strengths, and interests. Whether you need a little assistance or a lot, we’ll help you to live the life you want to lead.

What is supported living?

Our Supported Living service for adults with learning disabilities in Leeds enables you to live as independently as possible, in your own home and as part of the local community. We can provide support for as little as a couple of hours a week, right through to 24 hours a day.

The support we provide is led by you, giving you control over the service you receive and when you receive it. It could involve help to find a suitable home and manage your own tenancy, or it may be support with personal care tasks, daily living tasks or getting involved in your local community.

How we can support you

We know everyone is different, so we take the time to get to know you and the people that are important to you, such as your family and other professionals involved in your care. Together we develop a support plan that’s right for you and helps you to develop your skills, whilst also enriching your day to day life.

To do this we look at ten outcome areas:

  • Your health
  • How you spend your time
  • Being responsible
  • Being safe
  • Money and letters
  • Living skills
  • Communicating
  • Feeling good
  • People you know
  • Mental health and other conditions

Looking at these areas together helps us understand how best to support you. We use this information to design a support plan that’s unique to you and meets all of your needs. Once you are happy with your support plan, the service can start.

Putting you in control

We encourage you to do as much as you can for yourself. If there are things you can’t do independently, we will explore ways we could help you to do them, or do things on your behalf whilst encouraging you to participate as much as you are able. We listen to what you want, dealing with things according to your choices and decisions. By supporting you in this way, we ensure that you are empowered to have control over your life and develop your own independent living skills.

 

We can help you to do things like:

  • Find the right home for you
  • Manage your tenancy
  • Manage money and letters
  • Housework, cooking and cleaning
  • Personal care
  • Shopping
  • Taking medication and visiting the doctor
  • Using public transport
  • Socialising and hobbies
  • Going to work or college

…and, most importantly, whatever you want to do!

Why is supported living important?

Supported living enables adults with learning disabilities to have greater control and freedom over their lives, whilst ensuring they have enough support to meet their individual needs. It enables them to do the things that have purpose and meaning for them. These may be things that many people in the community take for granted, like choosing where to live, who to live with and how to spend their time.
