Giving you a hand to get back on your feet

Our Hospital to Home service for older adults in Leeds ensures you are discharged from hospital safely. We give you as much support as you need to help you get back on your feet whilst building confidence, independence and reducing the chance of hospital readmission.

We currently support hospital discharge from St James’s University Hospital, Leeds General Infirmary, Seacroft Hospital, Wharfedale Hospital, Bradford Teaching Hospitals and Bradford Royal Infirmary.

How does it work?

We will work closely with you, your family and other professionals involved in your care to ensure you have all the support you need to return home in a safe and supported way. This may involve working alongside doctors, occupational therapists, social services, district nurses and the hospital discharge team.

The type of support we provide will depend on your needs and preferences. We can visit you on your hospital ward to discuss how you’d like to be supported, speak with you on the phone or, if you prefer, make the necessary arrangements with your family. This ensures everything is in place for when you return home, giving you confidence and peace of mind.

Before going home

We will ensure everything is arranged for you, from making sure you have appropriate clothes to wear when you’re discharged, to ensuring your home is suitable for you to return to. This could involve putting the heating on, cleaning and tidying, making up your bed and doing a food shop. We can also arrange for home adaptations and installation of special equipment, such as handrails, key safes and community alarms, to make sure your home is safe for you.

Going home

We understand returning home after a stay in hospital can feel daunting, so we’re here to make the transition as easy as possible. We can collect you from hospital and stay with you until you feel settled. This could include making dinner, supporting you with personal care, staying with you through the night or providing round the clock live in care.

Ongoing support

We’re here to make life a little easier for you, so you can focus on your recovery and getting back on your feet. Your wellbeing worker can provide rehabilitation support, Home Help and Housekeeping, Personal Care, Companionship, Medication Support or specialist care for conditions such as dementia and Parkinson’s Disease.

Our home from hospital service is tailored to your individual needs and can incorporate any of our other services to ensure you have all the support you need to continue living well in your own home.
